Verification of 501(c)(3) tax exemption status
Organizational Structure
- List of officers and directors (show diversity spread - age, gender, race)
- List of key staff members and job descriptions for those responsible for project
Financial information - Project Budget, Agency Budget, most recent Audit
An application signature from the agency director will certify grant funds will not be used to support or engage in terrorism or discrimination against the LGBTQ community.

If your agency is funded, you will be expected to attend First Congregational Church's Mission Mixer held in the fall. This is an opportunity for you to highlight your agency and its accomplishments and give the church members an opportunity to meet you and learn about your work in the community.
